Please print and complete application. Your application must be completely filled out or it will not be processed.  You must also have at least one valid e-mail address in order for us to verify permission to obtain personal information.

Please include telephone number to Personnel ONLY on your application. Telephone numbers  provided for direct supervisors or which ring to your desk are not acceptable and will result in automatic rejection of your application.  If you are self-employed, you must have been in business for two years, provide copies of your last two years of filed taxes, and copy of a current bank statement.

You may fax the application and two most recent pay stubs back to us at (888)856-7339 or bring them with you to your viewing appointment along with a  $45/per adult application fee ($60/married couple).  The fee is payable in cash, certified check, or money order, and is non-refundable regardless of application status. We are unable to accept credit card payments at this time.  

For your convenience, you may also pay your application fee at any Wachovia Bank. Please call for details.
